A fetish is normally defined as a pathological fixation on a body part or object.
In kink a fetish is normally treated as means of obtaining sexual gratification.
Enabling a person to enjoy their special desires is surely healthier than forcing them to conform to conventions. The good or bad of it is mostly a matter of being able to successful satisfy the need. The fetish may be damaging because the person is too inhibited to act it out. True of even the most mundane sexual desires.
Becoming overwhelmed by a fixation is most likely the result of frustration or more general mental health problems, not the desire itself.
People with extremely dangerous or impossible fetishes are probably better off trying to eliminate the cravings.
