Pretty easy really.
Poly means many.
Fetish if anything has acquired too many meanings. Originally it was a term from ethnology for objects with totemic power among primitive peoples. Psychology grabbed it to make it an unhealthy fixation on an object a person requires for sexual satisfaction. Nowadays it shows up as just another word for things people are awfully keen about.
For me it signifies just about anything atypical that gives a person an erotic thrill. Issues of mental well-being are a matter of the individual and how he handles this pleasure. There are many gradations between bad sexual addiction and sane gratification.
For me a fetish is an act, practice that gives me a sexual thrill.
A fetishist is a person who enjoys many such fetishes.
A polyfetishist is a man or woman who has many such special route to erotic satisfaction.
